Eliminating Health Disparities in Diabetic Retinopathy Care
Community health workers are frontline public health personnel and trusted members of the community they serve. They are effective in improving health outcomes, health care utilization, and addressing health inequities in multiple chronic diseases including diabetes but are not often employed in the context of eye care in the US. The purpose of this study is to establish a pilot program to integrate a community health worker to address adverse social determinants of health among patients with diabetes who seek care in an outpatient ophthalmology office to improve vision outcomes.
